Employee Benefits

Pinnacle Hospital offers a bountiful array of insurance benefits to our regular staff members who have been offered a fulltime position (sixty (60) or more hours per bi-weekly pay). Insurance benefits become available on the first day of the month after thirty (30) days of employment

Pinnacle Hospital also offers a generous Paid Time Off program to our regular staff members who have been offered a part-time position (forty eight (48) or more hours per bi-weekly pay). It starts to accrue on the first day of employment and is available after successfully completing the ninety (90) day introductory period.

Health and Prescription Plan:

United Healthcare Choice PPO plan has been chosen to provide our staff the freedom and access to a network of physicians, facilities and other healthcare professionals, including specialists without designating a Primary Care Physician or without having to obtain a referral. There is no deductible or co-pay for preventive services, free home testing for diabetes and cholesterol, as well as a Fitness Reimbursement credit of up to $240 per year per member enrolled.

We cover 90% of the staff member cost and 70% of the added cost if you choose
to add dependents.

Dental Plan:

MetLife Dental Options PPO plan has been chosen to provide our staff the choice of dentists, no waiting periods once enrolled and no deductible or co-pay for preventive dental services, basic services are covered at 80% and major services (including orthodontia) is covered at 50% up to an annual maximum.

Vision Plan:

VSP Choice plan has been chosen to provide our staff the largest network of vision care providers, including some retail chains. There are no identification cards to worry about and no claim forms to fill out. Plan members have very low co-pays for the yearly exam, yearly lenses or contacts and bi-annual frames. Discounts for laser vision correction and specialty items.

Life Insurance:

Met Life has been chosen to provide our staff life insurance at one times the annual earnings up to $100,000 plus a flat $15,000 policy.

Pinnacle Hospital pays 100% of the premium for Life insurance coverage.

Met Life also provides Voluntary life insurance at very reasonable rates for additional employee coverage and dependent coverage.

Short & Long term Disability Insurance:

Standard Life Insurance Company has been chosen to provide staff coverage in the event of a disability. Short term disability begins on the 15th day of disability and can last up to 13 weeks. Long term disability begins on the 90th day and can last until permanent disability.

Pinnacle Hospital pays 100% of the premium for Disability insurance coverage.

401(k)

Lincoln National
has been chosen to provide staff a contribution program
with over fifty pre-tax investment options for retirement. Roth or traditional
IRA’s are available.

Paid Time Off (PTO) Program:

Pinnacle Hospital offers paid time off programs that includes vacations, holiday and personal time off as well as an extended illness benefit (EIB) for staff members offered part-time positions of forty-eight (48) hours or more per bi-weekly pay. This begins to accrue immediately upon hire. These accruals are available for use after successful completion of the ninety (90) day introductory period. The accruals listed below are based on 80 hours per pay period, those working 48-79 hours per pay are pro-rated according to hours worked.

PTO includes holidays, vacation time and personal time off and accrues as follows:

1 - 3 years of service   6.77 hours per pay   176 hours / 22 days per year
4 - 6 years of service   7.69 hours per pay   200 hours / 25 days per year
7 - 9 years of service   8.62 hours per pay   224 hours / 28 days per year
10+ years of service    9.23 hours per pay   240 hours / 30 days per year  

EIB includes illnesses lasting longer than five scheduled days and accrues as follows:

1+ years of service       1.54 hours per pay     40 hours / 5 days per year

Holidays

Pinnacle Hospital observes the following six traditional holidays and these days are paid at time and one half for staff members that are required to work:
• New Year's Day, January 1
• Memorial Day, Last Monday in May
• Independence Day, July 4
• Labor Day, First Monday in September
• Thanksgiving Day, fourth Thursday in November
• Christmas Day, December 25
